Entry Forms
Participants who DO NOT reside in Alaska, California, DC, Florida, Georgia, Guam, Illinois, Minnesota, Missouri, Mississippi, Nebraska, Tennessee, Texas, or Vermont may use the general entry form. Otherwise, find your state entry form below. International students should also use the general form.
If you are mailing your artwork in, please fill out the online entry form, download and print the finished form, attach the form to the back of your artwork along with your creative writing, and mail. Contestants from outside of the United States are not required to submit a creative writing and should upload a digital version of their artwork.

Group Form Instructions
Download the Student Information Sheet: Fill out all required details for each student.
If you do not have Excel, you may upload the file to Google Sheets to edit.
Upload the finished Student Information Sheet to the correct online entry form. Both mail-in and online submissions must fill-out an online form.
Artwork Submission:
If mailing in artwork: Use the Mail-In Name Sheets to write the student's name, grade, and fish species on the back of each piece of artwork along with each student’s signature. Print the finished entry form and mail with your students’ artwork and creative writings.
If submitting online:
Artwork: Scan each students artwork and save each file with the student’s name, grade, and species in the file name (e.g., "JohnSmith_Grade4_Trout.jpg").
Essays: For student’s grade 4 and above, save each essay file with the student’s name, grade, and “essay” in the file name (e.g., "JohnSmith_Grade4_Essay.jpg").
Uploading Artwork:
For scanned artwork and essays, gather all files in a folder. Name the folder the same as the school or studio being submitted from (e.g., "RasberryStudios-StudentEntries").
Zip the folder and upload it to the entry form, along with the completed Student Information Sheet.
